I've had really good experiences at Basko for full skin exams and biopsies and freezing. The doctors and PA's are very thorough and explain the process very well. However, when they have done biopsies in the past, they explain that if the results are positive, then the cancer would need to be surgically removed. I've had 3 biopsies and only 1 has come back positive, but in each case the ONLY option they describe is surgery. My most recent biopsy came back positive, and once again they only suggested surgery as the option. They pushed hard to schedule the surgery right away, even though they said there was no rush. In the meantime, I researched radiation as an alternative and after meeting with the radiation team at Endeavor. I chose that option. UPDATE 10/7/26. Thank you Dr. Basko for your thoughtful remarks, as a result of better understanding your practice's thought process, I have made some changes to my review. I have great respect for your team as I mention at the beginning of the review. I should add that the radiation physician did cover both the advantages and disadvantages of radiation over MOS and you are correct in several respects. One BIG negative to radiation is the need to make 20-30 trips to Edward. While they are very short sessions, the fact I only live 10 minutes from Edward and have a lot of time on my hands was a factor as well. If I lived 2 hours away, I definitely would not have chosen that path. I am knocking off 1 * because at my 6 month skin exam followup recently, after 10 radiation treatments, the Dr. (who was very thorough with the exam) once again emphasized that MOS is their recommended solution. The fact that she would say that when I was already in the middle of the radiation treatments was unsettling and led me to get a third opinion. The third opinion strongly confirmed the second opinion. And Dr. Basko is correct, you can get 20 second opinions and they can all be wrong. I simply think it would have been better if the Dr. would have accepted the fact that I chose the alternate treatment, as I sort of suspected she wanted me to stop the radiation and move ahead with MOHS, but I am not sure of that. And if the patient brings up radiation, simply state why, in that patient's specific case, one method over the other is recommended using the guidelines Dr. Basko describes. In the end I think we are lucky to have more than one option. I hope I don't have to make this decision ever again, even though there's some probability that I will, given the recurring nature of SCC. I am happy, right now, that I don't have a scar and hope I don't suffer complications from radiation down the road. I also want to emphasize the excellent care that I have always received at Basko dermatology and the professionalism of the entire team.

We fully support every patient’s right to research their diagnosis, ask questions, and seek second or third opinions before making a treatment decision. That said, we believe it is important to clarify several statements in this review. Mohs micrographic surgery is not a treatment that we recommend casually. When our physicians recommend Mohs surgery, the recommendation is based on the patient's pathology, the type and location of the skin cancer, its prognosis, and established evidence-based treatment guidelines. Mohs is widely regarded as the treatment offering the highest cure rate (99%) for many skin cancers, while simultaneously preserving the greatest amount of healthy surrounding tissue. Radiation therapy can certainly be an appropriate treatment for selected patients or skin cancers. However, it is not generally considered preferable to surgery simply because it is a nonsurgical alternative. Major dermatology guidelines continue to describe surgery as the cornerstone of treatment for several skin cancers. Radiation is more commonly considered when surgery is contraindicated, or when surgery may result in unacceptable functional or cosmetic consequences. We also respectfully disagree with the suggestion that obtaining two opinions recommending a different treatment necessarily makes those opinions more correct or our recommendation inappropriate. Medical expertise is not determined by who offers the most appealing alternative. The relevant question is which treatment provides the best expected outcome for the specific cancer and patient based on the available evidence. Regarding our efforts to contact patients after a skin cancer diagnosis, our intention is not to pressure anyone into having a procedure. When a biopsy demonstrates a cancer, our team follows up because untreated skin cancer can continue to grow and, depending on the diagnosis, may have major health consequences. Ensuring that a patient does not inadvertently leave a skin cancer untreated is part of responsible medical care. Our providers remain committed to recommending the treatment they believe provides the best combination of cure rate, safety, tissue preservation, and long-term outcome based on the available medical evidence—even when another treatment may initially appear less invasive. Ultimately, it is up to the patient to decide and we fully support their decision.
